As we grow older, chronic diseases, such as high blood pressure, cancer, diabetes, etc. also have a higher chance to develop. In order to improve access to healthcare for the vulnerable and underserved community, BPSOS is partnering with Office of Minority Health to implement the Chronic Disease Management program.
CHRONIC DISEASE
WORKSHOPS
BPSOS frequently hosts chronic disease workshops where we invite medical professionals to present on how to prevent and manage certain conditions for older adults.
REFERRALS
BPSOS also established an e-referral system where we have been connecting medical providers and non-profit organizations together. The purpose of this system is to increase our local vulnerable and underserved communities access to better health care and coverage.
